High-touch team-based care

Our cardiometabolic health program is tailored to each patient, with a uniquely proactive and personalized approach based on the latest science and delivered through telehealth.

Medication management

Using the latest society guidelines, we prescribe and titrate medications like ARBs, statins, and GLP-1 agonists. We support adherence through the Miga app.

Insurance navigation

Want someone else to do prior authorizations? We provide insurance navigation for patients to help them get the medication they need — and to take work off your plate.

At-home data monitoring

We provide access to validated blood pressure monitors and scales that are connected to the Miga app, which allows patients to store and share historical readings. Our clinicians use these insights to adjust treatment plans in real time.

Access to a dietitian

Every patient’s Miga care team includes a registered dietitian to help them with food choice, calorie tracking, and more. We also provide guidance on physical activity.

Coordination of care

Our job is to complement the full spectrum of services you provide. When your patients enroll with Miga, we communicate with you to coordinate their overall care.

What states do you operate in?

We currently operate in California, Colorado, Connecticut, Delaware, District of Columbia, Florida, Georgia, Illinois, Kentucky, Maryland, Massachusetts, Michigan, Nevada, New Jersey, New York, North Carolina, Ohio, Pennsylvania, Tennessee, Texas, Utah, Virginia, Washington, West Virginia and Wisconsin.

Do you take insurance?

Yes. We’re covered by Medicare, Blue Cross Blue Shield, Humana, Anthem, Aetna, and Sana in certain states, and we’ll be adding more insurers in 2024. You can find a list of insurance plans we accept here. Patients can use their insurance for clinical visits, lab tests, and medications that might be part of their personalized cardiometabolic health plan.

Is there an out-of-pocket expense?

Yes. Patients are responsible for any copay or coinsurance, just as they would be for an in-person visit.

What is unique about your solution?

At Miga, we provide comprehensive care for obesity, high blood pressure, diabetes, and high cholesterol, combining expertise in cardiometabolic health with a proactive, high-touch approach enabled by telehealth. We offer medication management, insurance navigation, at-home data monitoring, and access to a registered dietitian. What guidelines do you follow?

We design our cardiometabolic health programs based on guidelines from leading organizations like the American Heart Association, the American College of Cardiology, and the American Diabetes Association.

What documentation do you send back to me?

We’re flexible in the documentation we can provide on the patients you refer. We recognize that individual physicians may have distinct preferences, and we’re more than happy to meet them. We can share information about your patients via email or fax, but our clinicians are also available to discuss patients by phone when helpful.

What types of medication do you prescribe?

Using the latest society guidelines, we prescribe and titrate medications like ARBs, statins, and GLP-1 agonists. Most of the medications available through our program are generic, but we provide insurance navigation for medications that require prior authorization.

We support medication adherence through the Miga app.

How do I submit a referral?

To submit a referral, fill out our patient referral form. We recommend that you discuss Miga with patients prior to submitting a referral. If you have any questions about the referral process, or if you would like to request Miga brochures for your clinic, please email referrals@migahealth.com.
